PSHP and Rx Part D Eligibility Guidelines
While there are some plan sponsorship and residential state filing restrictions, beneficiaries insured with Premier Senior Health Plan can self-refer to any health provider nationwide that accepts Medicare. Part D Rx coverage is also available nationwide.​

ELIGIBLE GROUPS
  • Eligible groups include Corporations (C or S); Proprietorships; Partnerships; Unions; Government entities (Schools, municipalities, etc.); Non-profit organizations. All entities must have a Federal Tax ID Number.

ELIGIBLE MEMBERS

Eligible members include:
  1. Retirees age 65 and older enrolled in Medicare Parts A & B in any size employer group.
  2. Actives age 65 and older (also enrolled in Medicare Parts A & B) in an employer group with fewer than 20 total full and part-time actives of all ages.
  3. Current and surviving spouses or domestic partners at least age 65 and enrolled in Medicare Parts A and B. 
  4. Retirees/employees not eligible for Medicaid

Eligible members may enroll in PSHP at:
  1. Plan inception
  2. Within 30 days of retirement
  3. Upon aging-in to Medicare

ENROLLMENT PROCESS

Sponsoring Entity will need to complete the following: 
  1. Employer Participation Agreement, 
  2. United American Insurance Company Application 
  3. New Group Information Form 
  4. Companion Life Application for Supplemental Prescription Drug Expense Insurance. (Retiree only)
